When you arrive for your appointment you will be asked to provide information about yourself and your insurance coverage. It is important that you provide us with accurate information, so we may bill your insurance accordingly.
If you provide insurance information, we will file your insurance claim for you. Once your insurance company has paid, you will receive a bill that will reflect any remaining deductible, co-payment or patient responsibilities. If you do not have health care coverage you will be asked to pay a portion of your estimated charges at the time of service. You may contact Customer Service to discuss available payment options. Our Customer Service Department will be happy to help you complete insurance forms and provide any other assistance you need to settle your account.
