Thank you for choosing us for your health care needs. The information in this section is designed to help you understand your bill and answer some common billing questions.

When you arrive for your appointment you will be asked to provide information about yourself and your insurance coverage. It is important that you provide us with accurate information, so we may bill your insurance accordingly.

If you provide insurance information, we will file your insurance claim for you. Once your insurance company has paid, you will receive a bill that will reflect any remaining deductible, co-payment or patient responsibilities. Questions concerning these fees should be directed to the physicians' billing office Customer Service Department at 904.202.1032.

If you do not have health care coverage you will be asked to pay a portion of your estimated charges at the time of service. You may contact Customer Service to discuss available payment options. Our Customer Service Department will be happy to help you complete insurance forms and provide any other assistance you need to settle your account.

For your convenience, our providers accept payments by Visa, MasterCard, and Discover. Additional information regarding bill payment is provided in the Paying My Bill section.
