If you are visiting our office for the first time, please print these forms, complete them and bring them with you during your first visit.
Patient Appointments
All patients are seen by appointment. We recognize that your time is valuable and make every effort to keep a schedule, but we ask for your understanding that emergencies and lengthy visits may occasionally delay a physician. We will let you know if we are running late and give you the option of rescheduling your appointment if you choose.

Important Notices |
When we enter your information into our computer we use your name and date of birth as displayed on your insurance card.
Please use your name as it is printed on your insurance card when having a prescription filled or having any tests done. If not this will cause a delay in processing your claims, refilling your prescriptons and in processing your test results.
Treatment of Minors (persons under the age of 18) We cannot treat your child if you are not with them.

- Informing the receptionist about the nature of your visit when you make your appointment, so the proper amount of time can be allocated.
- Arriving 10-15 minutes before your appointment.
- Have your insurance card and picture ID out and ready to present to the receptionist.
- Bringing all of your medications or a list of your medications to your appointment.
- Notifying the office 24 hours in advance if you are unable to make your appointment.*
* There will be a $30 charge for a no-show of an appointment or cancellation of an appointment without a 24 hour notice. Your insurance company will not cover this charge.
Arriving late for an appointment
Patients who arrive 15 minutes late may be asked to reschedule their appointment.
Telephone calls
Please allow 48 hours for the physician or ARNP to review your message and chart. The medical assistant will return your call after the provider has addressed your questions.
Medication Refills
- To ensure more timely refills, please contact your pharmacy with the prescription number on your medication bottle. If your prescription has expired, the pharmacy will contact our office for permission to fill your prescriptions. Since 48 hours are required, please call before the last day of your supply.
- For your protection, an examination may be necessary before refilling some medications. A written prescription is required for controlled substances.
- Written prescriptions for mail order or prescriptions for the Naval Air Station:
All written prescriptions will be picked up for the patient to mail in or we can mail the written prescriptions to you.
Labs, Testing, Procedures and X-Rays
Our staff will notify you as soon as possible if any of your test results require prompt attention. Otherwise, you will receive a report summarizing your test results in the mail in two to three weeks. In some cases, the doctor may request that you schedule an appointment to discuss the results of your tests.
Referrals
Some medical problems require a referral to a specialist. We will do our best to accommodate your needs. Some plans require that you see specialists who are part of their network. Obtaining a referral is occasionally a time-consuming process and your patience is appreciated. Our referral coordinators will mail your referral to you, they will also fax the information needed to the specialist you will be seeing. Please call and schedule your appointment after you receive your referral. Please allow 10 -14 business days to receive your information in the mail.
Forms
For completion of forms ( FMLA, APS, Disability , etc. ) by the physician, the charge ranges from $15 (simple, one-page form ) to $25 (multi-page form or dictated letter). Payment is due at the time the form is submitted; please allow 5-10 business day for completion, after which time it may be picked up during regular office hours. We do not fax these forms.
KNOW YOUR RIGHTS and YOUR BENEFITS
The Florida Patients' Bill of Rights and Responsibilities is a law to help patients understand their rights concerning their health care. This Bill of Rights also explains how patients are responsible for knowing the benefits of their specific health plan. Although most services are covered by insurance, if you are not sure of your specific benefits for your health plan, call the customer service number listed on your insurance card.
Billing
We participate in most insurances plans/ however, we advise patients to check with their insurance company to confirm coverage. We ask that all patients sign a copy of our financial policy. Our billing is managed by a centralized billing office. If you have any questions regarding your bill, please call 904.202.1032.
After hours Care
An on-call nurse and physician are available for telephone consultation regarding urgent health conditions after during the evenings on weekends and holidays. You may contact the after-hours staff by dialing our office number, 904.778.3200. The on-call physician will not fill controlled substances or antibiotics.
