New Patient Forms If you are visiting our office for the first time, please print these forms, complete them and bring them with you during your first visit. Insurance Information Privacy Notice Health Care Status Authorization Authorization for Use of Answering Machines Medical Records Request or Release Forms For each form (FMLA, APS, Disability, etc.) that we complete and sign, the charges range from $15 (simple, one-page form) to $25 (multi-page form or dictated letter). Payment is due at the time the form is submitted; please allow five to seven business days for its completion, after which time it may be picked up during regular office hours.
