About Your Physician

Erich L. Schramm graduated from George Mason University in Fairfax, VA with a degree in Biology. He obtained his medical degree from the University of Virginia School of Medicine in Charlottesvill, VA in 2000 and moved to Jacksonville for his Family Practice residency at St. Vincent's Medical Center. He entered the Baptist Primary Care circle in 2003 and has been practice at the Lane Ave. center for the past 6 years.

Dr. E. Schramm's special medical interests are Sports Medicine, joint injections, dermatological procedures and managing the complex medical patient.

Rashmi U. SchrammRashmi U. Schramm graduated from the University of Virginia in Charlottesvill, VA with an undergraduate degree in Biology. She obtained her medical degree from the University of Virginia School of Medicine in 2000 and completed her residency in Family Practice at St. Vincent's Medical Center.

Dr. R. Schramm's medical interests are Preventive medicine, women's health, travel medicine and Pediatrics.

Erich and Rashmi Schramm met and married in medical school in Virginia and completed residency together in 2003. They are outdoor enthusiasts and enjoy staying busy with their two daughters, ages 4 and 2.

If you are visiting our office for the first time, please print these forms, complete them and bring them with you during your first visit.

Appointments

We recognize that your time is valuable. Patients are seen by appointment. We make every effort to keep a schedule, but please understanding that emergencies and complicated visits may occasionally cause delays. Our staff will keep you informed and give you the option of re-scheduling your appointment if that works best for your. Same day acute-care appointments are available.

  • Informing the receptionist about the nature of your visit so the proper amount of time can be set-aside for your appointment
  • Arriving 15 minutes early
  • Bringing all of your medications or a list of your medications to your appointment
  • Notifying the office at least 24 hours in advance if you are unable to make your appointment.*
    * There maybe a $30 charge for missed appointments and insurance will not cover this fee.

Late Policy

Patients who arrive more than 15 minutes late for their appointment may be asked to reschedule their appointment or wait until they can be worked into the existing patient schedule.

Telephone Calls

Our medical support staff has been trained to answer most questions. If they are unable to answer a question, your medical record will be reviewed and a staff member or physician will call you back.

Medication Refills

We do not want you to run out of your medication. Please remember to call for medication refills during regular office hours. To ensure more timely refills, please call your pharmacy with the prescription number on your medication bottle. If your refill has expired, the pharmacy will call our office for permission to fill your prescription. Please allow 24 - 48 hours for refills.

For your protection, an appointment may be necessary before refilling prescriptions for some medications including antibiotics and narcotics.

Labs, Procedures and X-Rays

Our staff will notify you as soon as possible if any of your test results require prompt attention. You will be called only if you have an abnormal lab or x-ray result. Otherwise, you may receive a report in the mail in 7-14 days summarizing your test results, depending on office policy. In some cases, the doctor may request that you wait until your next scheduled appointment to discuss the results of your tests.

Referrals

Some health conditions may require us to refer you to a specialist. You should be aware of the referral policies of your insurance plan. It may limit you to seeing only specialists that are affiliated with your health plan's provider network. Obtaining a referral through your insurance plan or a specialist's office is occasionally a time-consuming process and your patience is appreciated. We will do our best to meet your needs.

Forms

For each form (FMLA, APS, Disability, etc.) that we complete and sign, the charge range from $15 (simple, one-page form) to $25 (multi-page form or dictated letter). Payment is due at the time the form is submitted; please allow five to seven business days for its completion, after which time it may be picked up during regular office hours.

Emergencies

After office hours, on weekends and on holidays, there will be an on-call nurse and physician available for telephone consultation regarding urgent health conditions.

Patients with emergency conditions may be instructed to go to the emergency room at Baptist Medical Center or to the nearest emergency room if the emergency is life threatening. If hospital admission is required, our inpatient physicians will provide care for you. If you are with an HMO or PPO, please follow your policy's guidelines for emergencies.

Hospitalization

All patients requiring hospitalization will be admitted to Baptist Medical Center located at Downtown Jacksonville, Beaches, Nassau, South or Wolfson Children's Hospital.
