MRI (Magnetic Resonance Imaging) services are available at Baptist Primary Care Lane Avenue. After obtaining a physician's referral, you can call 904.202.2222 to schedule MRI services at this location.

Appointments
All patients are seen by appointment. We recognize that your time is valuable and make every effort to keep a schedule, but we ask for your understanding that emergencies and lengthy visits may occasionally delay a physician. We will let you know if we are running late and give you the option of rescheduling your appointment if you choose.
You can help us keep our schedule by:
- Informing the schedulers about the nature of your visit when you call, so we can allow the proper amount of time to be set aside for your appointment.
- Arriving 10 - 15 minutes before your appointment.
- Bringing all of your medications or a list of your medications to your appointment.
- *Notifying the office at least 24 hours in advance if you are unable to make your appointment.
* There is a $30 charge for a no-show of an appointment without a 24 hour notice. Your insurance company will not cover this charge. For your convenience we offer a 24 hour appointment cancellation line which you can access by dialing our main phone number (904.783.9680) and following the prompts.
Late Policy
Patients who arrive more than 15 minutes late may be asked to reschedule their appointment. All efforts will be made to reschedule appointments for the same day.
Telephone Calls
Please allow your physician the opportunity to review your message and chart. Either your physician, medical assistant or nurse will return your call during the day or the next business day.
Medication Refills
For prescription refills, please call your pharmacist and allow 24 - 48 hours for it to be filled. If your prescription has expired, the pharmacy will contact our office for permission to refill it. For your protection, an examination and/or lab work may be necessary before refilling prescriptions for some medications.
Labs, Procedures, Bone Densitometry and X-rays
We have a lab drawing station, X-ray facilities and offer bone densitometry in our office. Please allow at least 10 business days for results to arrive and be reviewed. Our staff will notify you as soon as possible if any of your test results require prompt attention. In some cases, the doctor may request that you schedule an appointment to discuss the results of your tests. Should you have your tests completed at a facility other than ours, please do the following:
- Request that the results be sent to our office.
- Inform us of the date and location of the testing so that we may follow up.
Referrals
Some medical problems require a referral to a specialist. We will do our best to accommodate your needs. Please be aware of the referral policies of your insurance plan, as some plans require that you see specialists who are part of their network. Obtaining a referral is occasionally a time-consuming process and your patience is appreciated. Our referral coordinators will mail your referral to you. Please call and schedule your appointment after you receive your referral.
After-Hours and Emergencies
An on-call nurse and physician are available for telephone consultation regarding urgent health conditions after office hours, on weekends and holidays. The on-call physician will not fill controlled substances or antibiotics. You may contact the after-hours staff by dialing our office number, 904.783.9680.
If hospital admission is required, our on-call doctor will provide care for you and notify us of your admission. Also, the on-call physician will provide care for newborns and will notify pediatricians of births. Please follow your insurance policy's guidelines for emergencies.
Billing
We participate in most insurance plans; however, we advise patients to check with their insurance company to confirm coverage. Before scheduling physicals, please make sure your plan will cover them.
