MEDICATION REFILLS - UNIVERSITY OFFICE
We do not want you to run out of your medication. Please contact your pharmacy for refills. You will receive a response within 48 hours. You will be given sufficient refills for maintenance medications to last until your next office visit. When you pick up your last refill you will need to schedule an office visit. You will need a written prescription for controlled substances.
The on-call physician will not fill controlled substances after hours or on the weekends.
KNOW YOUR RIGHTS and YOUR BENEFITS
The Florida Patients’ Bill of Rights and Responsibilities is a law to help patients understand their rights concerning their health care. This Bill of Rights also explains how patients are responsible for knowing the benefits of their specific health plan. If you are not sure of your specific benefits for your health plan, call the customer service number listed on your insurance card.
For example:
- Complete Physicals Exams are NOT covered by all plans
- Routine blood work is NOT covered by all plans
- GYN and PAP tests are NOT covered annually by all plans
These are just a few examples of the differences from one insurance plan to another. It is recommended each patient become aware of his specific benefits. Please call your insurance customer service representative today to answer any questions.
SPECIAL FEES
A nominal fee will be charged for the following:
- completion of forms
For each form (FMLA, APS, Disability, etc…) requiring a provider to complete and sign there is a charge from $15.00 (simple, one-page form) to $25.00 (multi-page form or dictated letter). The payment is due at the time the form is submitted. Please allow 5 - 7 business days to complete the form. We ask that you pick up the completed form.
- missed appointment
We request 24 hours advance notice for appointment cancellations. There is a $30.00 charge for missed appointments and insurance will not cover this fee.
